Jennifer Noble MSW, LICSW

Jennifer Noble is a successful Licensed Clinician who started her private practice in 2001, working mainly with older adults and families. She provides on-site services to Nursing Homes, Assisted Living facilities and Psychotherapy at home. She consults with Social Workers, Case Managers, Primary Care Physicians and Nurse Practitioners. In addition to working as a Psychotherapist, Jennifer owned and operated a temporary social work consulting company, providing Social Work Services to Nursing Homes.

She is a successful Entrepreneur, helping her clients feel safe and comfortable in working on changes in their lives.

Jennifer is a Licensed Independent Clinical Social Worker from Boston College. She also has a Certificate in Cognitive Behavioral Therapy and Couples Therapy. Her training and concentration at Boston College focused on Health and Medical Social work with training in Oncology and Cardiac Rehabilitation at Lahey Clinic in Burlington, MA. She also has intensive work history in treating Dually Diagnosed Traumatic Brain Injured patients assisting in the facilitation of Multimodality Therapy Groups emphasizing a Cognitive/Behavioral Treatment Approach.

Psychotherapist

Celeste Ricci-Trahan MSW, LICSW

Celeste has had an extensive background in the aging and health care field. Her thirty-plus years of experience in the health care field has included home care, acute hospital and long term care. Her expertise includes end of Life and Hospice care; as well as after-loss counseling.  She has provided clinical supervision in long term care and home health settings to both students and professionals.

After serving many years as a legal advocate for the Jeanne Geiger Crisis Center, Celeste then acted as Co-President for the Board of Directors.  She has also chaired the Program Committee for the New England Chapter of the National Association of Aging Life Care Professionals (formerly Geriatric Care Managers); as well as serving on the Board for several years.

Celeste provides Elder Care Consulting Services to consumers in Southern and Coastal New Hampshire; as well as the North Shore and Merrimack Valley areas of Massachusetts.

Celeste has recently partnered with Honoring Choices, a non-profit group based out of Massachusetts. The mission of the group is to provide consumer education around end of life care planning and the choices available.

Kristen received a Bachelor’s degree in psychology and sociology from Merrimack College and a Master of Social Work degree from Boston College.

She has an additional certification as an Advanced Hospice and Palliative care Social Worker and completed the certificate program in Primary Care Behavioral Health at The Center for Integrated Primary Care at UMASS Medical School. She also serves as a High Risk Care manager in Primary Care.

Psychotherapist

Patty Decotis, msw, licsw

Patty is a Licensed Independent Clinical Social Worker. She has a BS degree in Psychology and a Master’s degree in Social Work from Salem State University. Patty has an extensive background in providing mental health care for older adults, for adults needing end of life care, and has a passion for providing care for adults affected by disabilities and/or trauma. Her expertise comes from having been the Director of Social Services at several skilled nursing facilities, as well as from many years working for the PACE program helping adults to live in the community, successfully, for as long as possible.

Patty Provides talk therapy based on cognitive/behavioral theory with a perspective that is strength based and solution focused. Using empathy, compassion, and empowerment, goals are established that a client wants to work on in order to return to, or establish, a desired level of functioning. Patty’s desire to provide the best client centered care she can, has led her to researching cutting edge, evidence based, medical and mental health information.
